Surveys help us learn if we are doing a good job for you. Please be on the lookout for two surveys you may receive from The Centers for Medicare & Medicaid Services (CMS). One survey is the Consumer Assessment of Healthcare Providers and Systems (CAHPS), and the other is the Health Outcomes Survey (HOS).
CMS first distributes these two surveys by mail. For those who don’t respond by mail, CMS then reaches out by phone. CMS selects survey participants at random, so not all CarePlus members will get to participate each year.

CAHPS Results
This survey measures your experience with your primary care physician and CarePlus. CMS runs the CAHPS survey between March and June every year.
