What’s a D-SNP?

D-SNP stands for dual-eligible special needs plan. It is a special kind of plan that combines your Medicare and Medicaid benefits in one easy-to-use plan. It includes coverage for doctor visits, medication, dental, vision, and much more.

Medicare Advantage D-SNPs usually bring extra benefits that you wouldn’t get from Original Medicare or Medicaid alone.
 

Who is it for?

To enroll in a D-SNP, you must have both Medicare and Medicaid.

CareNeeds Plus (HMO D-SNP) and CareNeeds Platinum (HMO D-SNP): This plan is available to anyone receiving both Medicare and Medicaid: Qualified Medicare Beneficiaries (QMB/QMB+), Specified Low-Income Medicare Beneficiaries (SLMB/SLMB+), Qualifying Individuals (QI), Qualified Disabled and Working Individuals (QDWI) and other Full Benefit Dual Eligibles (FBDE). The CareEssentials Allowance may not be combined with any other allowance benefits nor used to purchase Medicare-covered prescriptions or services, nor converted to cash.
