Dental Benefits for CarePlus Members
Dental coverage is included on every CarePlus plan. If you are a CarePlus member, read this page for important dental benefit information.

Using our Dental Network

CarePlus members can get dental services covered under their plan from in-network general dentists or dental specialists (e.g., oral surgeon, endodontist, periodontist, etc.) who participate in the Florida GoldPlus Dental network, which is administered by HumanaDental Insurance Company. 

You do not need to get preauthorization or a referral before seeing a dental provider in our network for dental services covered by your plan.

You received your dental member ID card in the mail when you enrolled in a CarePlus plan with dental benefits. Your dental member ID card is different from your CarePlus member ID card. 

Be sure to bring your dental member ID card with you to your dental visits. When calling to set up a dental visit, mention that you get dental benefits with your CarePlus Medicare Advantage plan. You also may be asked to say the name of the dental network.

Use your secure MyCarePlus member portal account to access a digital version of your dental member ID card and for dental benefits and claims information 

You can also use your MyCarePlus account to access your other health plan information, including medical benefits and claims information.

How to File a Dental Grievance or Appeal

You can submit your grievance or appeal within 60 calendar days of the date of the event or denial notice. Your grievance or appeal should be sent in writing; however, we’ll accept phone submissions. Please call Member Services at 1-800-794-5907 (TTY: 711) or mail/fax your request to:

How to ask us to pay you back or to pay a bill you have received:

You may request us to pay you back by sending us a request in writing. If you send a request in writing, send your bill and documentation of any payment you have made. It’s a good idea to make a copy of your bill and receipts for your records. You must submit your dental claim to us within 12 months of the date you received the service or item.

To make sure you are giving us all the information we need to make a decision, you can fill out our claim form to make your request for payment.

You don’t have to use the form, but it will help us process the information faster. Send us your request for payment along with the following information:

  • Proof of payment– Itemized bill listing the item or service received
    • Dentist order (if applicable)
    • Dental records (if applicable)
    • Any other supporting documentation
