Using Your CarePlus Rewards Account

You can create and use a free CarePlus Rewards Account on the Sharecare website and app. An easy way to find your CarePlus Rewards Account is by going to your secure MyCarePlus member portal. After you first register at CarePlus.sharecare.com, you may continue using the website or download the Sharecare mobile app from the App Store® or Google Play®.

Go to MyCarePlus for your CarePlus Rewards account on Sharecare. In Sharecare, click the “Achieve” icon and then select “Rewards.” 

No. CarePlus Rewards is included as part of your CarePlus Medicare Advantage plan.

Earning Points

No. We added more activity options for 2024 to give you more ways to earn a gift card. You can choose any combination of activities that add up to 1,000 points.

You do not need to tell us you completed a healthcare activity. We will automatically receive that information once you complete the required steps outlined below. Please allow some time for points to be updated in our system.

Your doctor will submit health claim information to us that lets us know you completed these activities. You do not need to do anything else. It may take some time for your CarePlus Rewards account to show the points you earned. Please ask your primary care physician which of the eligible doctor visits and screenings are right for you.

You can complete the RealAge test electronically on Sharecare any time to earn points. The RealAge test has a lot of questions, but you don’t need to answer all of them at once. Sharecare lets you pause and go back to it later. Or you may call CarePlus Member Services to receive and complete the RealAge test by mail. Please keep in mind that mailing the test takes time, so please request it early or complete it online to avoid missing the chance to earn and redeem points before the end of the year.

These activities are limited to certain times and locations, and they require Sharecare registration to earn points. To earn points, you must scan a QR code or go to  certain webpage URL as indicated during these activities. Once you log in to Sharecare through the code or URL, you will automatically receive points if eligible. For support, you may ask for help at the event location or call Member Services. Please visit our webpages to learn more about CarePlus Link, Wellness Education, PXCircle, or CarePlus Community Centers. You may complete as many or as few of these activities as you would like, but the total number of times you can earn points for any combination of these activities is six times between Jan. 1 and Dec. 31. If you do not want to register for Sharecare or participate in this way, you may complete other rewardable activities listed on the Careplus Rewards page.

Certain slideshow articles are listed as rewardable in your CarePlus Rewards account on Sharecare. When you read or click through the entire slideshow, you will earn points for completion. This activity requires using Sharecare online or by app. If you do not wish to register for  Sharecare or participate in this way, you may complete other rewardable activities listed on the Careplus Rewards page.  

Quarterly steps challenges are available in your CarePlus Rewards account on Sharecare. New rewardable challenges start in January, April, July, and October, and each challenge is available across a three-month period, meaning you may join any time. Each challenge has a unique goal to earn a certain amount of steps, but you do not have to reach the goal to earn points. To earn points for participating, join the challenge in Sharecare and then just log steps toward the goal on at least two days during the challenge.

This activity requires using Sharecare online or by app. You may participate in as many or as few quarterly steps challenges as you want. If you do not want to register on Sharecare or participate in this way, you may complete other rewardable activities listed on the Careplus Rewards page.

Walking is not the only way to earn steps. You may use the following steps conversion chart to estimate steps based on time spent doing other types of activities.

To log steps in Sharecare, you can either connect a supported tracking device or manually enter your estimated number of steps. More challenge details are continuously added to our Healthy Habit Challenges webpage

Redeeming Rewards

You need to earn 1,000 points to redeem a $50 gift card.

Yes. We must receive your gift card order by Dec. 31. You must redeem your reward by this date, or you will lose your points.

Use your CarePlus Rewards account on Sharecare to check how many points you’ve earned during the year. You need 1,000 points to earn a reward. After you earn 1,000 points, we will notify you by mail, email, or text message and provide ordering instructions. You can also check your completed activities and points anytime on Sharecare.

You can receive one $50 gift card per plan year.

Ordering online is the fastest way to order and receive your gift card. Just use your  CarePlus Rewards account on Sharecare. If you do not use Sharecare to redeem your points, you will receive a letter with instructions to redeem by mail, fax, or phone. You will receive your gift card by mail if you select a physical gift card or by email if you select an e-gift card.

When you redeem, you can select a gift card from Walmart, Shell, T.J. Maxx, Macy’s, Home Depot, Darden Restaurants, Applebee’s, Chili’s, Cracker Barrel, Starbucks, PetSmart, or Denny’s. You can select an e-gift card or physical gift card. Please choose the option you prefer. 

When you redeem  1,000 points, you may choose an e-gift card that you receive instantly by email. This is the fastest option. 

After we receive and process your order, please allow 10 to 14 days for shipping through the U.S. Postal Service. 

No. All gift cards are shipped or emailed directly to you free of charge. 

No. Once you order your gift card, it is final and cannot be changed or cancelled.

In accordance with the federal requirements of the Centers for Medicare & Medicaid Services (CMS), no amounts on the gift cards shall be redeemable for cash and no amount may be applied toward the purchase of any prescription drug under your plan. Rewards (gift cards) must be earned and redeemed in the same plan year. Rewards not redeemed by 12/31 will be forfeited.
